What would be the best move on each street?

I believe I blundered this hand on every street because of Fatigue.

We are in a daily tournament 8 players left. Pays 3 spots.

I thought that the blinds were 400 800, 800 and intended to raise 3 big blinds preflop but they had gone up. What would be the ideal amount here?

On the Flop I make a continuation bet and I believe that is a mistake because the flop is too wet. Is that correct?

On the turn I have an Open end straight draw and I shove with a pot size bet left. Given that both opponents called the flop is there enough fold equity warrant this play? If I check I can still have chips if I give up on the river.
